• Links to digital health intake forms and practice policies

By booking an appointment you are agreeing to the following terms:

• You are over the age of 18 year old

• You will be located in the state of Colorado at the time of the appointment

• You do not currently receive Medicaid or Medicare

• This microclinic does not accept insurance. All fees are transparent including a detailed Good Faith Estimate which is shared at the time of booking. Payment is expected at the time of service.
